Basic Information

Generic Name: Semaglutide

Molecular Type: GLP-1 receptor agonist (94% homology with natural GLP-1)

Specifications: Core Mechanism of Action

Pancreas: Glucose-dependent insulinotropic effect, inhibits glucagon, provides stable blood glucose control with low risk of hypoglycemia.

Gastrointestinal Tract: Delays gastric emptying, enhances satiety, and reduces food intake.

Central System: Acts on the hypothalamus, suppressing appetite and reducing hunger.

Metabolic Effect: Improves insulin sensitivity, reduces visceral fat, and protects β-cell function.

III. Indications (Global/China)

1. Adults with Type 2 Diabetes

For patients with poorly controlled diabetes through diet/exercise, it can be used alone or in combination (e.g., metformin, SGLT-2 inhibitors).

For patients with cardiovascular disease/chronic kidney disease, it reduces the risk of major adverse cardiovascular events (myocardial infarction, stroke) and kidney disease progression.

2. Long-term weight management for obese/overweight adults

Obesity: BMI ≥ 30 kg/m²

Overweight: BMI ≥ 27 kg/m² + at least one weight-related complication (hypertension, hyperlipidemia, type 2 diabetes, OSA, etc.)

IV. Dosage and Administration (Standard Regimen)

Injection (Ozempic/Wegovy, once weekly)

Starting dose: 0.25 mg/week for 4 weeks (adaptation period)

Increase dose: Increase every 4 weeks, up to a maximum of 2.4 mg/week (weight loss); commonly used for diabetes is 0.5–1 mg/week

Starting dose: 3 mg/day for 30 days (adaptation period, no hypoglycemic effect)

Increase dose: After 30 days → 7 mg/day; for stronger hypoglycemic effect → 14 mg/day

Administration: Take on an empty stomach (30 minutes before meals), with only a small amount of water, swallow whole.

V. Core Clinical Efficacy

1. Hypoglycemic Effect

Significantly reduces blood sugar HbA1c, can be reduced by up to 1.5%–2.0%, superior to most oral hypoglycemic agents.

Very low risk of hypoglycemia (glucose-dependent onset).

2. Weight Loss (Milestone): Injectable 2.4mg/week, 68 weeks: Average weight loss of 15–18kg (15%–18% of initial body weight).

3. Cardiovascular/Metabolic Protection: Reduces the risk of major adverse cardiovascular events (MACE) by approximately 26%.

Improves blood pressure, blood lipids (lowers triglycerides, increases HDL), waist circumference, and reduces liver fat.

VI. Adverse Reactions (Predominantly mild to moderate, transient):

Common (30%–40%): Gastrointestinal: Nausea, diarrhea, vomiting, constipation, decreased appetite (significant in the initial stages of treatment, alleviates with adaptation).

Injection Site: Erythema, itching, induration (injectable).

Rare/Caution Required: Pancreatitis, gallbladder disease (low incidence, abdominal pain should be monitored). Hypoglycemia (dosage reduction required when used in combination with insulin/sulfonylureas).

Acute kidney injury (risk increased with severe vomiting/diarrhea and dehydration).
